The most-favored-nation contract was released on a Saturday by the consumer watchdog organization Public Citizen, which obtained it after filing a Freedom of Information Act lawsuit. The group also released documents related to the Eli Lilly contract, and the release provides additional detail about how the Trump administration’s drug-pricing policy fits in with broader trade negotiations. The timing is deliberate: Section 232 pharmaceutical tariffs take effect at 12:01 a.m. Eastern time on September 29, 2026, for all companies not already listed in Annex III, which took effect July 31, 2026. The two pieces of policy are connected, and PFE sits at the center of both.
The Business
Pfizer is a $60-plus billion revenue company navigating one of its more complicated stretches. In its second-quarter 2026 report, Pfizer posted adjusted diluted EPS of $0.77 and revenue of about $15.0 billion, and it raised full-year 2026 revenue guidance to a range of $60.5 billion to $62.5 billion. Non-COVID products grew 5% operationally, with PADCEV up 23% year over year, and launched and acquired products generating $3.2 billion, up 18% operationally. That is the growth story. The complication is the structural weight accumulating around it.

That opacity is a problem for investors trying to model the deal’s impact. What is clear is the intent. The White House has framed the policy as a way to ensure that incremental revenue from higher prices abroad benefits American patients rather than drugmakers.
The tariff angle adds another layer. Companies that execute both a Commerce-approved onshoring plan and an HHS MFN pharmaceutical pricing agreement can qualify for a 0% additional duty rate through January 20, 2029. Pfizer signed its MFN deal to reduce tariff exposure, which is rational. But the price of that protection is now visible: it is unclear which part of the government receives the shared funds or how, specifically, they will be deployed, beyond language that they are intended to lower costs for U.S. patients and taxpayers.
What’s Driving the Opportunity
The bull case for PFE has not changed with this disclosure. CEO Albert Bourla has emphasized the dividend and expressed confidence in high single-digit revenue growth from 2029 to 2033, citing multiple upcoming regulatory decisions. The pipeline is real, oncology is producing, and PFE recently traded around $27.66. The average 12-month analyst price target sits at $28.88, with the high estimate at $35.75.
For investors focused on PFE specifically, the MFN contract covers existing marketed drugs rather than pipeline assets, which means the drugs most investors are already valuing as growth drivers may carry less policy overhang than initially feared.

What Could Go Wrong
The redacted revenue-sharing rate is the variable that cannot be priced. If that percentage is material, every overseas price increase Pfizer negotiates over the next few years generates less operating income than the headline number implies. Pfizer has also warned that loss of exclusivity and related pressures are expected to weigh more meaningfully beginning in 2026. A hidden government claim on international pricing power adds friction to a period when the company needs that lever to work cleanly.
Peers with less MFN exposure, including Merck, Lilly, Johnson & Johnson, and AstraZeneca, do not all carry the same disclosed share-back language. Public Citizen also released records from an Eli Lilly agreement, while other agreements with numerous drugmakers have not been released publicly in full. Until those contracts surface, the competitive comparison is incomplete.
The Bottom Line
Pfizer is ownable at current prices, but with a tighter thesis than it appeared to have before the weekend. The MFN contract does not destroy the investment case; it qualifies it. Every dollar Pfizer captures by raising prices in Europe or Japan now has an unknown government claim attached. The tariff relief Pfizer can qualify for is real and worth real money. What investors are paying for is a business where a meaningful slice of the international pricing upside may flow to Washington rather than to earnings per share.
